Severe Acne is one of the commonest conditions I treatments offered at the Specialist Skin Clinic. A large number of the patients suffer with severe disfiguring acne which can be quite challenging to treat. Often patients with severe acne also suffer with acne scars. Patients are often keen to find out what gets rid of scars. Unfortunately scars are permanent and can be improved but not removed. For this reason the best treatment for scars is to have appropriate treatment of acne early enough so that scars never appear.

Patients often want to know what is the number one cause of severe acne and the answer is always the same. Acne is a genetic condition. This means it tends to run in families. The way acne is passed on through families is not clear because only recently has focused work on the genetics of acne been started.

Accepting that you have acne not because of anything you have done to your skin but rather because one or more of your family members may have had it before you removes the very common question of what actually triggers severe acne. There are many myths and unproven theories of too many chocolates or of make-up acting as triggers for acne. There may be some truth to these but no direct proof offered that acne is anything other than a random occurrence based on your family history.

Patients often ask ‘how can I clear acne?’ or ‘how can I get rid of acne? The good news is that even the most severe cases of acne can be successfully treated and indeed cleared. The drug which has had the greatest impact on the treatment of acne is a retinoid known as oral isotretinoin.

Oral isotretinoin  is extremely effective in treating all severities of acne. However because of the side effects associated with this treatment it is often used in patients with severe, scarring acne or in patients whose acne has failed to clear after using a number of other treatments.

Oral isotretinoin has multiple effects on the sebaceous glands which are the structures in the skin which secrete sebum resulting in the oily skin that is associated with acne. These glands shrink in size during the course of treatment.  The drug dries the skin and slowly gets rid of acne spots. This treatment will result in clear skin in the majority of patients on this drug.

Oral isotretinoin is the most important drug used in the treatment of severe acne. The treatment offers the possibility of completely clearing the skin. In a significant number of patients this improvement is long lasting. For many patients with severe acne this drug can be life-changing.  In the UK this drug can only be prescribed by a dermatologist.
